In Santa Monica, California, Dr. John Form gives his future wife, Mia, a rare vintage porcelain doll as a gift for their first child, to place it in a collection in their daughter's nursery.

That night, the couple is disturbed by the sounds of the next-door neighbors, the Higgins, who are murdered during a home invasion. While Mia calls the police, she and John are attacked by the Higgins' killers. The police arrive and shoot one of the killers, a man, while the female killer commits suicide by slitting her throat inside the nursery holding the doll. The news identify the perpetrators as the estranged daughter of the Higgins, Annabelle, and her boyfriend, both members of a cult.

In the days following the attack, a series of paranormal activities occur around the Forms' residence. Then, Mia gives birth to a healthy baby girl, whom they name Lia. The family moves to an apartment in Pasadena. After finding the doll (which John had thrown away after Annabelle's previous attack) in one of their boxes, another series of paranormal events plagues Mia and her daughter.

Mia calls the detective who handled the original case and learns that the cult intended to summon a demonic spirit. With the help of her bookseller and roommate Evelyn, Mia realizes that the cult practiced devil worship, which summoned a demon that followed the family after they moved into their apartment, in order to claim a soul.

Returning home, Mia and Lia are attacked by the demon which manifests while handling the doll. Mia and John contact their parish priest, Father Perez, who explains that demons sometimes attach themselves to inanimate objects to achieve their aims and that a human soul must be offered for some purpose. Unable to exorcise the demon, Father Perez decides to take the doll to seek help from the Warrens. But before he can make it into the church, the demon masquerading as the spirit of Annabelle attacks him and snatches the doll.

Father Perez is hospitalized, and when John visits him, the priest warns him that the demon's real intent is to claim Mia's soul. That night, while Evelyn visits Mia, the demon uses Father Perez's physical form to slip into the apartment and kidnap Lia for her mother's soul. To save her daughter, Mia attempts to jump out the window with the doll, but John and Evelyn arrive in time to stop her. Evelyn decides to give her life in Mia's place, as atonement for causing a car accident years earlier that resulted in the death of her daughter Ruby. As Evelyn lies dead in the street, the demon and the doll disappear. Mia and John never see or hear the doll again and Lia is safe in her crib.

Six months later, the doll is purchased from an antique shop by a mother as a gift for her daughter Debbie, a nursing student, and is seen stored and locked in a glass case in the room with the Warrens' artifacts.
